Attention Parents of children who live in Fort Zumwalt School District and will be 5 years old by July 31, 2023.
What: Kindergarten Registration
When: 2-7 p.m., March 1, 2023

WHAT YOU'LL NEED
Your child's birth certificate --- Your child's immunization records --- Your valid driver's license, state or federal photo ID. --- You also need one document from List A below, as well as a second document from List A or List B.
List A
Current Rental Agreement
- Must be supplied if renting
- Signed by landlord & tenant(s)
Current Mortgage Statement
If you just purchased a home, recent ALTA Settlement Statement*
-Must be signed by buyer & title agent
Current Gas or Electric Bill
-Mailing & service address must match
Affidavit - required if living with another family, friend, significant other who owns/leases home
List B
Current Bank Statement
Current Credit Card Statement
Current Loan Statement
Current Water Bill
-Mailing & service address must match
Welfare, Social Security, or Other Official Mail from Federal & State Agencies
Affidavit - Homeowner must supply document from List A. Parent must supply document from List B.
All documents need to be dated within the past 30 days
*If you moved in the last 2 weeks, a second document from List A or from List B will need to be supplied in 30 days
